Children and Young People's Service: Home Care and Adolescent Transition Care
We provide extremely caring, professional and approachable staff to work with children and young people with a range of complex special needs such as emotional behavioural difficulties (EBD), physical disabilities and associated sensory, communication and learning difficulties and/or autism and behaviour that challenges. We offer the following children/young people’s services:
Domiciliary care - personal care
Social inclusion support – accessing a range of activities
Sitting respite service
Supported living and transition to adult services for young people who need on-going care. Through person-centred Planning we will listen to the views of the person in relation to their individual education and training needs and leisure needs to ensure that the transition to adult services is progression, not setback.
16+ semi-independent living services
Our 16+ Service provides a high standard of accommodation and supportive environment for residents and unaccompanied asylum-seeking children aged 16 to 24 years. We aim to ensure young people are able to develop and maintain respect, self-reliance and self-care skills and enable them to make informed, age-appropriate decisions, concerning their present and future so they can achieve a successful transition into independence and adulthood. Services offered are based on an assessment of the young person’s needs.
Currently, we provide this service in Northamptonshire and are in the process of setting up services in Leicester, Leicestershire, Sheffield and other areas.
